The status quo

Today an eligibility check returns the parameters of a benefit: coverage active, $640 of a $2,000 deductible left, 20% coinsurance, no copay. Everything except what the visit will cost. A patient who has not met the deductible, or whose benefits are more complicated, gets one of the most confusing and frustrating experiences in healthcare.

A richer price for the Intake Agent

Avon already estimates patient responsibility during intake. Superscript could enrich that step with the services the visit is predicted to bill, the practice's own adjudicated rates, and an explanation of each part of the patient's cost share, including what changes when the encounter changes.

We specialize in patient cost shares. Once the practice's claims history, payer mappings and models are configured, step 6 can request an encounter price: deductibles, coinsurance and all their combinations, not just the copays the industry stops at. Same trigger, same downstream steps.

The full financial picture

Pricing a visit takes three engines, and they do different jobs. AIM predicts what the encounter will bill as, from the practice's own appointment and claims history. ABE predicts the allowed amount for that practice, that provider and that payer. AIR reads the benefit and applies it to those rates. An eligibility feed is a part of the third. Supplying all three for a specific practice, provider and payer is the work.

What actually happened

Medicine is emergent. At checkout the encounter is recorded as delivered and repriced as it is recorded. The services have to reach the integration for that to happen at the desk; otherwise the change is detected later, from the claim.

Whatever checkout recorded is what the patient is shown: the services actually billed, the amount charged, and how it compares with the price they agreed five days earlier.

The claim goes out

When claim data is available, the services actually submitted are compared with what was predicted. What Dana paid does not move: the payer has not decided yet, and a paid patient invoice is not an adjudicated claim.

Reconciled against adjudication

The remittance is compared line by line with the price Dana paid. In this example the payer agrees and the balance is zero, so there is nothing to chase and nothing to refund. When it does not agree, the difference is shown by line.

WHAT THE API RETURNS

One request per appointment returns the price, the benefit that set each line, and what could still change it before the visit.

patient_responsibilityOne number for the encounter. In cents, for the base reality of the visit, ready for the chart, the portal or an invoice.
base.items[]One row per code. Each priced on its own, so the amount is made of parts a person can read rather than a single opaque total.
adjustments[]Which benefit set each amount. In words a care navigator can read out, so a cost question stops becoming a payer phone call.
x_axis[]What the visit may bill as instead. Each variation carries a swaps pointer to the line it replaces and its own delta.
y_axis[]What may be added on the day. Priced marginally against the base, so a delta can be shown without repricing the encounter.
insurances[].accumulatorsWhere the patient stands for the year. What is left of the deductible and the out-of-pocket maximum at the moment the price was set.
